If you have the email, click on ‘Get Reward Form’, confirm your information and an individualized form will pop up just for you. This rebate is limited to one per household and you have to have received the email to participate. Purchases must be made between 7/9/2010 and 7/23/2010. Form must be submitted by 8/24/2010.
